Meeting the Unique Needs of Older Adults

Horizon Health’s senior care programs serve adults aged 65 and older, providing specialized care and therapeutic programming. Research has shown that older adults are often less comfortable seeking care from a mental health professional than their younger counterparts. Due to stigma, misinformation and false beliefs about aging, they frequently go without adequate care for depression and other psychiatric illnesses and psychological problems.

Horizon Health strives to promote successful aging and help your patients maintain positive psychological experiences that will keep them socially engaged and improve cognition and physical function. Together, we can help your aging population improve their overall quality of life.

What we offer

Specialized Care

  • 24-hour nursing and medical support
  • Medical management, psychiatric diagnosis must be primary
  • Individualized treatment planning
  • Diagnostic testing, if applicable
  • Family education and support
  • Basic life skill enhancement
  • Recreation and / or occupational therapy
  • Discharge planning
  • Individual and group therapy
  • Motor skill training
  • Behavior management techniques

Therapeutic Programming

  • The unit provides at least 35 hours of groups / programming a week, including weekends and holidays.
  • Daily therapeutic programming includes the following at a minimum:
    • Goals group
    • Group therapy sessions
    • Occupational / recreational / activity groups
    • Patient education groups
